The purpose of this form is to serve as a waiver of liability and a covenant not to sue. This form shall serve as a standing agreement and covers a period beginning on the date the form is signed and continuing through August 31st of the following calendar year.

In consideration for permission for me and/or my children to participate in children and youth activities hosted on the property owned or controlled by Keystone Community Church, a Michigan ecclesiastical corporation ("Keystone"), I agree to release, discharge, and convenant not to sue Keystone or any of its employees, agents, or volunteers, for any claim of loss or damage or any claim as a result of any injury or loss of life (the "Claims") sustained by the undersigned or by his/her children from the children and youth activities, or any activity in, on or about the property, whether or not due to the ordinary negligence or omission by Keystone, its employees, agents or volunteers, or other children or participants. I accept full responsibility of any and all risks associated with my child's attendance and participation in the activities discussed above. I agree to indemnify, defend, and hold harmless Keystone and any of its employees, agents, or volunteers from any liability resulting from any Claims or from my child's presence or my child's partipation in any such activity.

I consent to both first aid and emergency medical attention in the event of my child's injury or incapacitation while participating in such activity of at the property, and agree to release and defend, indemnify and hold harmless Keystone, its employees, agents and volunteers from any damages or liability resulting from the provision of first aid or emergency medical attention including movement and transportation to a medical care facility.

I am the legal guardian of the child(ren) listed and, to the best of my knowledge, have legal authority to accept terms of this waiver and consent form on my child's behalf.
